Ladder Safety Checklist

Purpose: Ensure safe use of ladders in the workplace to prevent accidents and injuries.

Instructions: Before using a ladder, complete this checklist. Mark each item as complete or incomplete. Address any incomplete items before proceeding.

Pre-Operation

  • Inspect Ladder:

    • Ensure the ladder is free of oil, grease, and other slipping hazards [1].
    • Check for structural defects such as broken or missing rungs, cleats, or steps [2].
    • Ensure the ladder is not painted with any substance other than a transparent coating [3].
    • Verify that the ladder is clean and free of substances that may cause slipping [4].
  • Placement and Stability:

    • Place the ladder on a stable and level surface [1].
    • Secure the ladder to prevent accidental movement [5].
    • Ensure the ladder extends at least 1 meter above the landing surface [5].
    • For non-self-supporting ladders, use at an angle where the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length [1].

During Operation

  • Usage:

    • Maintain a three-point stance on the ladder at all times [5].
    • Do not extend any part of the body beyond the side rails [5].
    • Face the ladder when climbing or descending [4].
    • Use at least one hand to grasp the ladder when climbing [6].
    • Do not carry objects that could cause loss of balance [6].
  • Restrictions:

    • Do not use the top two rungs of a portable ladder unless it is a stepladder with a platform and handrail [5].
    • Do not use ladders on slippery surfaces unless secured or provided with slip-resistant feet [1].
    • Ensure ladders are not used near exposed energized electrical conductors [5].

Post-Operation

  • Storage and Maintenance:
    • Store ladders in a dry location away from excessive heat [7].
    • Support ladders in storage to prevent sagging [7].
    • Tag and remove from service any ladder with defects until repaired [2].

Corrective Actions

  • Document any issues found and actions taken to correct them.
