PPE Compliance Safety Checklist
This checklist is designed to ensure that all employees are using and maintaining personal protective equipment (PPE) correctly, in accordance with safety regulations and best practices. It covers key areas such as hazard assessment, equipment suitability, training, and maintenance. Use this checklist to verify PPE compliance and identify areas for improvement.
Hazard Assessment and Equipment Selection
- Has a workplace hazard assessment been performed to identify the need for PPE? 2, 17
- If no, conduct a hazard assessment to determine potential hazards requiring PPE. 2, 17
- Is there a written certification that the hazard assessment has been performed? [1]
- The certification should identify the workplace evaluated, the person certifying the evaluation, and the date(s) of the assessment. [1]
- Are the selected PPE types appropriate for the hazards identified in the assessment? 2, 17
- Verify that the PPE protects against specific hazards such as impact, penetration, compression, chemical, heat, harmful dust, and light radiation [2]
- Has the employer communicated PPE selection decisions to each affected employee? 2, 17
- Is PPE of safe design and construction for the work performed? 2, 13, 17
- Does the PPE properly fit each employee? 2, 17
- Ensure PPE is suitable and a proper fit for each worker [3]
PPE Provision and Usage
- Is approved PPE supplied to workers at no cost to them? 1, 9
- Is the required PPE at the worksite before work begins? [3]
- Are workers wearing the personal protective equipment that is correct for the hazard and protects them? [4]
- Are workers properly using and wearing the personal protective equipment? [4]
- Do workers use footwear appropriate to the hazards associated with the work being performed and the work site? [5]
- Is personal protective equipment used to meet the requirements of 29 CFR Part 1910, Subpart I? [6]
Training and Information
- Are employees adequately trained in the proper use and care of PPE? [7]
- Do workers receive training in the correct use, care, limitations, and assigned maintenance of the PPE? [4]
- Are workers informed of the reasons why the PPE is required and the limitations of its protection? [3]
- Are employees trained to know when PPE is necessary? 2, 13, 17
- Are employees trained to know what PPE is necessary? 9, 13, 17
- Are employees trained on how to properly don, doff, adjust, and wear PPE? 9, 13, 17
- Are employees trained regarding the limitations of the PPE? 9, 13, 17
- Are employees trained on the proper care, maintenance, useful life, and disposal of the PPE? 9, 13, 17
- Do workers know the location of the personal protective equipment and are trained in its use? [3]
Maintenance and Storage
- Is the personal protective equipment maintained and kept in a sanitary condition? 1, 13, 17
- Is the personal protective equipment stored in a clean, secure location that is readily accessible to workers? [3]
- Is personal protective equipment inspected for damage prior to use? [8]
- Is there a regular system of inspection and maintenance of personal protective equipment furnished to workers? 13, 17
- Is personal protective equipment tested or visually inspected before each use, in accordance with the manufacturer's specifications? [7]
- Is defective or damaged personal protective equipment removed from use or service? 1, 2, 13, 17
- Are workers taking reasonable steps to prevent damage to the personal protective equipment? [3]
Specific PPE Requirements
- If a worker's eyes may be injured or irritated at a work site, does the worker wear personal protective equipment to protect the eyes that is approved to CSA Standard Z94.3-15, Eye and face protectors , and appropriate to the work being done and the hazard involved? [4]
- Where a person is exposed to a hazard that may injure the person's head, is protective equipment worn that is appropriate to the hazard and that complies with 1 of the following standards: the latest version of CSA standard CSA Z94.1, 'Industrial Protective Headwear'; or the latest version of ANSI standard ANSI Z89.1, 'Industrial Head Protection'? [9]
- Where a person is exposed to a hazard that may injure the person's foot, is protective equipment worn that is appropriate to the hazard and that complies with the latest version of CSA standard CSA Z195, 'Protective Footwear'? [9]
- Where a person is exposed to a respiratory hazard that may cause injury or disease, does the employer provide and ensure the use of an adequate respirator that is appropriate to the hazard? [9]
Corrective Actions
- Are defects in personal protective equipment repaired immediately? [3]
- Is personal protective equipment replaced immediately if it cannot provide the protection it was intended for? [3]
- If personal protective equipment is returned by a worker due to defects, is it immediately repaired or replaced? [3]
