To prevent recurrence of similar incidents and ensure regulatory compliance, investigate the following:
1. Fall Protection: Ensure all personnel in the platform connect their fall protection device to a fall protection anchor before raising the platform. Attach only one fall protection device to each anchor. The Personal Fall Arrest System must only be used by personnel who are trained and authorized to do so, under the supervision of a qualified person. All personnel must be connected to an approved anchorage at all times, while using a double leg. 100% tie-off, shock absorbing lanyard that limits arresting force to 900 pounds. All personnel must use the fall restraint system while in the platform. 2. Dropped Object Prevention:
- Implement a dropped object prevention program that includes securing tools and materials to prevent them from falling from the aerial lift. This may involve using tool lanyards, tool bags, or netting around the platform.
- Conduct regular inspections of tools and equipment to identify and replace any worn or damaged items that could contribute to dropped objects. 3. Aerial Lift Safety Procedures:
- Reinforce pre-start inspections and workplace practices to identify potential hazards. Survey the work area for stationary or moving obstacles and surface hazards such as holes, drop-offs, bumps, curbs, or debris; and avoid them. Never strike or bump into stationary or moving obstacles while driving or raising and lowering the platform.
- Ensure operators are trained and qualified to operate the specific type of aerial lift being used. The operator bears ultimate responsibility for following all manufacturer's instructions and warnings, regulations and safety rules of their employer and/or any state or federal law.
- Never operate the machine without all parts of the guardrail system in place and the gates closed. Make sure that all protective guards, cowlings, and doors are securely fastened. 4. General Safety Rules:
- Verify that all placards and decals are in place and legible before using the machine. Never use a machine that is damaged, not functioning properly, or has damaged or missing placards and decals.
- All platform occupants must wear a fall restraint or fall arrest device connected to a fall protection anchor as required by national or local regulations and standards.
